Clinical outcomes and effectiveness
Measuring progress
At Ellern Mede, clinical effectiveness is reviewed through regular multidisciplinary team (MDT) reviews, informed by the individual’s formulation and supported by both standardised outcome measures and personalised goal-based outcomes.
Outcome measures are selected according to age, presentation and individual formulation. They are typically completed at admission, key review points (including CPA meetings) and discharge.
Progress is reviewed collaboratively with patients, families and community teams with the aim of supporting sustainable recovery and improved quality of life.
Outcome measures – adults
For adults, routine measures may include:
