Following a comprehensive assessment, we develop an individual care pathway that brings together specialist staffing, therapeutic support, adapted environments and personalised recovery goals. Care is regularly reviewed with families, commissioners and community teams to ensure each individual continues to progress safely towards greater independence.
Planning for recovery
Recovery begins from the day of admission
Our focus is on helping individuals build resilience, develop independence and prepare for life beyond hospital. Through structured therapeutic support, regular reviews and collaborative discharge planning, wE work towards reducing restrictions and supporting successful transitions back into the community whenever appropriate.
Referrals
We welcome early discussions with commissioners and referrers to explore whether a Bespoke Care Placement is the right option.
Following a comprehensive clinical assessment, our team will determine whether we can safely meet the individual’s needs and develop a personalised package of care.
